Examples of acceptable grounds for appeals are shown below:

Unavoidable Delay:  Delays because of physical handicap, medical or family emergencies, military service, and other situations beyond a homeowner's control

Administrative Error:  Forms were sent to the sent to wrong address, the wrong form was sent, information was misplaced, and other such errors.

New or Additional Information:  Information which was not previously available is now available.

Mistaken Information:  An error in date of install, postmark or leak, type of leak or system caused a denial of benefits

Unusual Circumstances:  Other serious situations such as medical conditions or dangerous situations which deserve special consideration.
